When to start bottle feeding baby?

When should the baby be weaned of breast milk to bottle milk. How does one go about bottle feeding baby? Is there a mixture which to make bottle milk for babies. What is that mixture and what is the temperature the baby’s milk should be at? How often should the baby bottle be sterilized? Where can a first time mother see a diagram of how to feed a baby with a bottle? Are there safety measures to be followed, if yes what are they? Till what age does the baby drink from the bottle? Which is the best milk (product) to give to the baby?
